DLM Page Addon – some questions (issues)

I download the DLM Page Addon ( http://mikejolley.com/projects/download-monitor/add-ons/page-addon/ ) but the “theme” have some differences to the old version of the plugin.
How can I make this :
1) In the home page of the downloads (http://lurdesvieira.com/teste2/downloads/) and on the category (http://lurdesvieira.com/teste2/downloads/download-category/testes-intermedios/) don´t show the icon of the file type.
See this on the old version of the plugin (http://lurdesvieira.com/downloads and http://lurdesvieira.com/downloads?category=36 )2) The search box, only show on the home page of the download…
Can have the search box, on the category, subcategory and the download page. See this on the old version of the plugin.3) Show links to subcategory (on the category page). Example: http://lurdesvieira.com/teste2/downloads/download-category/exames-nacionais/ – this category (exames-nacionais) have some subcategory (Matemática A and Matemática B)
