Don't know why when I input the wrong CAPTCHA in the comment form, I would be direct to the following ERROR message page ? ANY solution?

My problem is not related to any particular plugin cause I've tried many different CAPTCHA Plugins.

"Error: You entered in the wrong CAPTCHA phrase. Press your browser's back button and try again.

    Internal Server Error

The server encountered an internal error or misconfiguration and was unable to complete your request. Please contact the server administrator..."

    * SIMILAR situation arises when I tried to delete a "POST" in dashboard, when I click "move to trash" button on top right, I'll be directed to the following page

"You do not have sufficient permissions to access this page.
    Internal Server Error ..."

BUT When I click "back" page, my post is successfully deleted !

it seems that whenever user done something their role don't get this permission, they will be directed to this "internal server error" page, and the first statement could clearly says the reasons, well, such as "you don't have the permission to load image", "Error: You entered in the wrong CAPTCHA phrase" ... etc.

    What's wrong ?
