Diamond Post Widget Issue

I’m getting this weird appearance of the post header when I try to use the following code
[diamond-post format='<span class="title">{title}</span> <span class="date"> {date} <span class="excerpt">{excerpt} {more}</span>' before_content='<div class="posts-wrapper">' after_content='</div>' before_item='<div class="front-posts">' after_item='</div>' min_post_count='4' count='4' date_format='M/d/Y' more_text='Continue Reading' post_limit='4'/]

If I use {title} in the format string, 2 repeating titles of the same post will appear. And if I don’t use that tag, post title is not going to appear. I uploaded a screenshot here. How can this issue be solved?
