I've created a custom post type and custom taxonomy and they work fine in the admin, the URLs appear to be correct, however whenever I try to view one, or an archive, or a category archive, I see the homepage:

I've tried to use twentyeleven to see if my theme was conflicting, but it makes no difference which theme is used. Also, this could be the same issue, but there are no body classes signifying that i'm looking at a custom post type. It's not a 404 error, it just displays the Static Front page template. What am I doing wrong?
Additionally, my taxonomy also doesn't show up in the Appearance > Menus page. It's missing the box so I can't add the categories to any menus.
