I want to make a custom login and registration form for use in my template. I have the login form code and my form style, but I can not find a similar registration form code that can be used. Is there a script available? All of the results I have seen so far take me to plugins, but I would like to use it direct in my template. If there is no code available for me to use, ideally with inline checking for username availability and captcha (ideally reCaptcha, as I use that in the site already and would like consistency), is there a plugin that can do the same that you would recommend? Any help would be very much appreciated.

Try out http://www.cozmoslabs.com/wordpress-profile-builder/ There's a free version as well so you can try that one. Should work just fine.
Also since it's shortcode based you can include it in your pages directly and style it as you want.
