Can someone recommend a plugin that would allow for business name, address, phone, email to be entered into a database and then a simple form where the user would enter a zip code and have all the entries in that database display that are located in that zip code?

Posted 5 months ago #I know this is late and I'm sure you've already found a solution, but for something simple you can always use the contact form 7 (http://contactform7.com/) in conjunction with the contact form 7 database extension plugin (http://wordpress.org/extend/plugins/contact-form-7-to-database-extension/) they worked great for me in a completely different application, but basically it is completely customizable so you can pretty much do whatever you want. Since this may be long after you found a solution, I left it for others who might search for a similar answer.
