Does anyone elses page look like this?
    Is it my theme?
And does anyone know how to fix it?
Do I need to hire someone to fix this layout or can someone help me fix it?

Can't the labels be on top of each field, and / or the editable area be wider. I've seen the label part fixed before, but I don't remember the coding to move it.

    Thanks for the help! -Brad

After no response, I've taken the page down. Not sure if this was a browser issue, a CSS issue, or what the deal was, but the plugin didn't quite cut it in my opinion.

Here's a screenshot of how it looks installed.

Again, not sure what the problem was, but it didn't look too good.

Probably your theme's CSS or another plugin's CSS overrided Frontend's CSS. Try using Firefox with Firebug installed to see what is happening with the styling in the affected fields.

The new development version has tightened up the CSS a bit to lessen this but this is a two edged sword as it detracts from the theme.
