Creating a form that routes data to different emails based on user selection (1 post)

I work for a non-profit agency that in turn works with numerous other non-profit agencies as partners. We want to put a form on our site to allow people to sign up for our services, basically it would gather some of the information we need and then our partners could contact the folks to get the last few bits.

I'm dealing with 23 counties here, though, and 10 places for the data to go. I could forward all the data myself but that might get difficult if we have a high volume. I'd like to make a form where depending on which of the 23 counties the user selected it would route the data to the correct person's e-mail automatically.

I've created the form in Form Manager. What I would like to happen is have the e-mail routed to the correct place depending on what county is selected. Here is the form if that helps give you an idea. It doesn't have all 23 counties in the menu yet.

Is this possible? Is there code I could add or options I could fiddle with in the form to make it work? I'm new to WordPress and any help would be greatly appreciated.
