Coupon Code problem with Cart66?

When someone uses a coupon code, the checkout form has alignment issues. The form’s fields show up after the box instead of before and the lines are all messed up.

Any idea how to edit this? I can’t find a place to adjust settings for the contact form in the checkout.

I’m now going through and deactivating all other plugins to see if there’s a conflict.
