Couple of bugs

Hi – just to report a couple of bugs just in case you weren’t aware:

1. On the Payments addon, if questions or checkbox options are long, the visuals break a bit when selecting payment by field

2. It would make sense to have a “Duplicate” option on the form selection screen as well as within a form, because that may be where users like me look for it!

3. When using radio/checkbox buttons, one doesn’t always use a title as the options sometimes speak for themselves. In these cases, VFB just shows a blank space, when maybe it should create something descriptive from the option values? Just a thought. 🙂
