I have tried using intouch for its awesome power of the form but for some reason I can't get it to work.

Can anyone give me a similar solution to what I'm seeking? Here's an example of exactly what I want.

Yeah, try WP-ContactForm by Ryan. You should be able to extend it to suit yourself pretty easely.

I would love to try out that plugin, but I have no idea how to extend it so that all those extra fields exist since I'm unfamiliar with PHP. Is there an easy way to do this?

Yes there is, contact the author and offer to pay to have it done for you. Otherwise, try the wp-pro paid service.

I think you could probably do that fairly easily with the contact form from dagon designs:

I never was provided a solution to my problem unfortunately. Is there anyone else out there that can help a PHP noobie to fix this problem?

Your were provided with the best solution that could be offered for a (self proclaimed) "php newbie".
The link above your last post - by vkaryl - takes you to a very customizable contact form: no PHP knowledge, just reading required!

I don't see any error - except it's ugly because you cannot fit it in the space you have :)

The error I'm getting is this one:

No connection to the mailserver. Please try again later.

I have no idea where to change this value or how to make it work.

The plugin page has more than 400 comments... This issue has been asked (and answered) several times - so, do your homework and read.
Also, make sure sendmail is enabled and working on your server.

thanks moshu. i found the answer which wasn't clear in the original instructions. I think I'll use this solution assuming they like it. Thanks for your observations.
