Contact Form that supports multiple recipients

    I was wondering if anyone knows of or can recommend a Contact Form plugin for WP that supports multiple recipients (ideally through a drop down menu, rather than having to have multiple contact forms on a page).
    It would be great if it also supported akismet/captcha or had some form of anti-spam measure.
