Contact form shows admin email and name not blanks for user to fill in

Client has contact form plugin from bestwebsoft that was downloaded automatically via the Go Daddy. I’ve tried to add the contact form to a number of times to pages but every time I do regardless of the settings the contact form shows the admins username and email address in the From and Name fields when the page is viewed live I’ve tried changing the main settings in the contact form without any success.

There is a contact form to DB version and using that has not solved the problem.

Can anyone recommend a fix or another contact form plugin that connects to a DB without this problem. Free or not – I’m looking for the best solution that I an also use on other sites.
