Could you give me more details about what happens when you click on the button? Do you receive an error? Does the form builder disappear?
Could you also try to deactivate all other plugins, one at a time, to make sure that none of them conflict with Jetpack?
If that doesn’t help, could you open your browser’s Javascript console, and let me know if you see any errors when you click that button? You can find out how to open your browser’s javascript console here:

Hi Jeremy, nothing happens when I click the button, the only way out is to close the pop up and then it asks you if you are sure because the changes will not be saved.
Javascript Console is giving the following when I click the button:
POST http://northluffenham.com/wp-admin/admin-ajax.php 500 (Internal Server Error) jquery.js:2
When I turn off Photodropper http://wordpress.org/plugins/photo-dropper/ the button problem is cured 🙂
I can confirm; I installed Photodropper, and I could reproduce the issue.
I would suggest that you contact the plugin author here, and let them know about the issue. The problem might be linked to the way they insert jQuery with their plugin; this page provides more information about this topic.

FYI
I had this exact same issue and I do not have Photodropper installed. I do however have Social Gallery. Disabling that sure fixed the problem.
