I just installed Contact form 7 and it's malfunctioning:
1. "generate tag" drop down menu is missing
2. The short-codes for each form are missing, right under "Copy this code and paste it into your post, page or text widget content." - There's nothing there
3. When I click on "Add new" it places the correct url in the url bar but I don't get the usual pop up about the language and it doesn't take me anywhere. I can only "edit" and "copy" the current forms.
I have installed the same plug in on other websites, same WP version and this is the only one that is messing up.

Have you tried:
- deactivating all other plugins to see if this resolves the problem. If this works, re-activate the plugins one by one until you find the problematic plugin(s).
- switching to the Twenty Eleven theme to rule out any theme-specific problems.

I am having the same problem. No Contact Form icon, no generate tag, not even a default form. I am running wordpress 3.4.1 and the latest version of contact form 7 on a debian server. It is missing something that the plugin relies on?

Guys think I found the problem. For my situation, the problem had to do with symbolic links. The path to some essential scripts were incorrect as a result of a different structure in the filesystem via symbolic links. I am guessing it had to do with php/wp function calls such as dirname( __FILE__ ) or locate_template(). Check if Symbolic links are present on your server and debug whether or not they are causing these problems.

I have found that the Easy Content Types plugin (http://pippinsplugins.com/easy-content-types/) is causing this conflict/error with "Contact Form 7" for me, but I don't know how to fix it.

same here.
used to work but tags have disappeared altogether.
I played with "Front end editor" and "TinyMCE advanced" in the meantime but have disabled them since then with no luck. Tags have still gone walk about...
