First, identify what contact form plugin you are using and then post in that plugin’s dedicated forum via its page in the Plugin Repository.

@bradborland: The shortcode information isn’t exactly helpful in identifying the plugin responsible. You need to look in Plugins > Installed Plugins and find the one that powers the contact form. Then you can post your issue in the proper forum.
I have Jetpack and Pushpress. I don’t know if that helps or not. Nothing specifically for the contact form…
