I am looking for a plugin that allows users to make a comment and provide a 1-5 rating. All the plugins I have found only do the rating system. I want a way to attach the rating to the comment so users can see what the person wrote and why they gave it the rating they did.

This plugin does the trick. You simply have to turn on "Comment Author Ratings" (see documentation on usage)
