   I have the "Allow Comments" boxed check marked for the pages where I'd like to allow comments to be left and not checked for the pages I don't want comments. Everything works fine except on 1 page. I have a check mark in the box, but no comment form is displayed. Anybody have this situation and know how to fix it?

   Does this page use a custom page template?

   Are you using a custom page template for this page/post?

   You seem to be missing the call to <?php comments_template(); ?> for that page/post.
