Comment blacklist not stopping spam

I have Akismet and I also use the comment blacklist to block some IP’s and include certain keywords. However, I still keep getting the same spam (from an IP I blocked and including the keywords I put on the blacklist). It ends up in the spam folder, but why isn’t Akismet totally blocking it? Is there a way to let Akismet know that this really is spam?
