Code for simple opt-in to receive pdf

    Greetings all,

    I am looking for some simple code to capture name and eMail for people who wish to receive a free pdf document. I can find optin plug-ins to send eMail or sign-up on a mailing list, but I really just want to redirect the visitor to a hidden page with links to free links. Any suggestions?
