Client Management System & Data Capture Form

Hi,
I’ve done my research both here and google but can’t find exactly a thread on what I’m looking for. I’m looking to put a basic form at the end of a post which advertises a different specific item each day, I want readers to be able to request more info via a short form at the bottom which then emails to the individual product’s company email and sends a copy to me. I also longer term want to integrate this with a Client Database for essentially collection of all leads. I’ve done some basic research such as Zoho and SugarCRM (any user reviews would be good) but how easy is it to integrate with a wordpress form or does anyone know of a simple plugin that translates data captured from a form to a simple CSV worksheet.
Different products will feature each day so I need a form that I can quickly adapt to include a new product email address,a heading which tells them what product they have requested info on and obviously doesn’t show the reader the email of the product being reviewed.

I’m sure this exists as there are tons of business blogs out there I just can’t find this in the plugins or support sections ?
