client centered query

I have been looking for a plugin or something I can use that my client can pull information from database.

Here is what I need:
– Client selects information from dropdown choices and/or enters information like a test score.
– Client presses submit
– A report opens showing the results from the query.

I created the process as a Access Database report but I need to move it online so the client enters the information.

Is there a plugin or something that I can use for this? Suggestions
