Clean-Contact plugin for Contact Form

    I am having trouble getting Clean-Contact to work on my site. I followed the instructions here exactly:

    http://wordpress.org/extend/plugins/clean-contact/

    Unfortunately, once I activated the plugin, nothing happened, and there are no further instructions. Is it supposed to just work by itself, or should I be using a widget and pasting in code? It does not show up as an option in my Widgets. Any help would be appreciated.
