Checkboxes not displaying correctly… HELP!

First off, love this plugin! Simple, clean and to the point. Only issue I have is the checkboxes are not in front of the text but rather it looks aligned to the right on the line above the question.

I’ve contacted my theme developer but he is not sure.

And how would I make the < Back and Next > buttons actual buttons? My theme has pretty cool looking buttons but how would I add that specific style to the existing button?

Is there a way to change the style of the question so they are larger and maybe a different color?
