Cascade forms

I’ve been googling a lot to find a form plugin that but I can’t find anything. It has to allow to add as many sub-forms as the filler needs. The purpose of this is that the leader of a family can fill his personal data, and then add a subform for maevery family member, and finally send the data into one email message.
