It could very well be caching. If they refresh and see the same image then it is a caching issue.

The plugin requires that PHP be configured with the GD graphics extension and PNG creation enabled. This is standard for most hosting companies. It will not work without it.

This was a caching issue. I just added the login/join page to the list of un-cachable pages in W3 Total Cache. Marking as resolved!

Do you mean even I need to install “W3 Total Cache” plugin to resolve this issue..???

No, the fix is only if you need to use w3 total cache. If you use any caching plugin you may have to list wp-login.php and wp-comments.php as not cached.
If you don’t use a cache, then this is not an issue for you.
