Hi mbrsolution. Thanks for the URL. I looked at it and tried a few things, but nothing was fixed. The icon for a CAPTCHA image shows up, and there’s a tag on it that says CAPTCHA image. it just doesn’t show a CAPTCHA. I try to refresh it, and no change. Looking forward to another suggestion! Thanks!
Hi edibletriangle what theme are you using? Have you tried a WordPress default theme like Twenty Thirteen?

Hi edibletriangle I had a look at your URL and could not see any captcha entries in the code. Have you removed the captcha settings?
Also can you test the default thene I mentioned above. I am trying to establish if it is a theme issue or not.

HI! I just tried Twenty Thirteen and had the same issue. It shows an icon in the captcha section, but there is no captcha.
Hi I checked your captcha image path and it does not look correct..

Thanks very much for your help. I think I’m just going to leave out the captcha image for now.

I have the same problem that the captcha image didn’t show at all.
I currently using the 4.0.21.
Checking the code I found this:
