Can't upload images with 3.4.1

I looked for an answer to this but could only find older questions. I don’t have any of the plugins those other posts mentioned either.

When I go to insert an image into a post the ‘add image’ window pops up the front page of the website instead of posting the image. This is a recent problem. I believe it was working fine prior to this latest update.

WordPress does this when I try to upload a brand new image or add an image from the media library.

Steps I’m taking:
I click the add media icon next to upload/insert.
The window pops up.
I either click the From Computer, Gallery, or Media Library tab.
After uploading or selecting an image from the gallery/library and clicking the ‘insert into post’ button, the window churns for a second and then pops up the front page of the website.
It doesn’t insert the image into the post but it does actually save it to the directory.

Any assistance would be appreciated.
