    Have you tried:

    - deactivating all plugins to see if this resolves the problem? If this works, re-activate the plugins one by one until you find the problematic plugin(s).

    - switching to the default theme to rule out any theme-related problems?

    - resetting the plugins folder by FTP or phpMyAdmin? Sometimes, an apparently inactive plugin can still cause problems.

    Hi,

I'm also have the same (if not similar) issue with WordPress 2.9.1. I don't believe it's a permissions-related issue. Please read on.

    Steps to reproduce:

    - login to the admin section, go to the dashboard.
    - under 'Posts' click 'Add New' link
    - place the cursor in the content field (either Visual or Html)
    - click on the 'Add an image' icon (upload window opens, 'From Computer' tab selected)
    - click the 'Browse' button, select image from drive.
    - click the 'Upload' button.
    - the 'Browse' file field is cleared but no image is shown below in the Upload Window. No new entry is visible in the Media Library as well even though the file has been uploaded to the server.

    More information:

As @daveredfern mentioned. Uploads with either quick posts or existing posts work. This only occurs with the 'Add New' link.

If I enter a subject first (before placing the cursor in the content field) the draft is saved once I lose focus on the (subject) text field. A permalink is then assigned and appears on screen. Once this happens (and I continue the steps above) the upload works as it should and I do see the file in the Media Library.

As I mentioned before the file IS actually uploaded. It's visible from both the file system on the server as well as through the browser. It's just not visible in the WordPress system. WordPress does not appear to save any information to the database upon completion of the upload process (the file is absent from the Media Library).

    Thoughts:
If it's a requirement that all uploaded files must be attached to a permalink or post ID when creating a new post (before it can be added to the Media Library) then the user probably shouldn't be allowed to upload one UNTIL this requirement is filled.

If the user should be capable of uploading files to the Media Library without first creating a post (which is do-able from the 'Media' > 'Add New' (from the left sidebar)) then this behavior is probably indicative of a bug or at the very least a UI oversight.
