Can't select answer in multiple choice and preview is different

    The preview looks good but when viewing it on the page there are the following errors:
    1. There are numbers on top of words
    Example:
    Ultimate Neurology Quiz
    1Question 1 of 12 (1 is on top of Q)
    Then 2, 3, 4…are on top of the multiple choice answers. Not sure why this is happening.

    2. Multiple choice answers cannot be selected (no radio button)

    Under the quiz title there is “Question 1 of 12” then “1. Question”
    How do I change “1. Question” to say something else or remove entirely.

    How do I make it so only one multiple choice answer can be selected per question?
