Can't Save

I just downloaded this plug-in successfully and started creating fields. Yet when i click “Save Form” it goes to a blank screen with only a “1” in the top left corner. When I go back to the plug in, all of the fields I put into the form are there, but nothing I typed in (field name, etc.) is there – it all went blank. So i am left with a series of blank fields. Please help – thanks!
