When I try to restore a revision by clicking on the "restore" link in the revisions meta box, I get redirected to /wp-admin/undefined
Has anyone else had the same problem? Thanks for your help!

Yeah, I just ran into the same issue. =/
FWIW, my current setup is: WordPress 3.5.1, WP Document Revisions 1.3.5, and Members 0.2.2 on Debian 6.0.
