I have only just installed this plug in myself but i think the issue maybe the space? (between 40 and questions)

Yeah, the shortcode should just go in the body.
have a look at the 40 questions category in the wordpress admin menu
Posts >> Categories and then click on the ’40 questions’ category
what is the slug name?
try putting the slug in
possible examples:

I realized that you actually dont need this plugin.
You can add a “category” page (kind of) as a sub-menu to a Page via the WordPress menu settings.
Makes sense?
It depends what you need to do really but you are right, you can create a sub-menu.
