Cannot Create Levels

I click on “Add New” and it takes me to “Edit Levels” Page with the Level Title pre-filled with the words “new level”. I change the title add some rules and scroll to the bottom to click “Add”. It takes me back to the Membership Levels page and there are no levels listed. I have tried several different configurations of this, i.e. not changing the title name, not supplying any rules, selecting associated roles etc… still no levels are being created. I have logged out, logged in, everything I can think of but it won’t create the levels.

Any ideas?

Using WP 3.5.1 and Membership 2.0.7
