   I want to give a page the address of http://www.mysite.com/forum or forum.php

   Is this possible? If so, how do I accomplish this?

   Create a page with the page slug you want to appear, and publish it. The slug gives the page URI.

   Thank you for responding.

   I tried what you suggested and have found that it has no effect on the page URI. Am I missing something?

   You need to set your permalinks (settings -> permalinks) choose something other than the default.
