Can we show seach form on a separate page

Hello Everone,

I want to add search form separately on a page and show the results to a page as it is in present condition. When a user will come to our website , he will be shown this store locater form at the footer of home page and as he fills the value and click on submit button should redirect to a page having results to that values with the map as it is now.

Please Suggest me ASAP, if it is possible..
