Call to action

Hi, I would like to know if there is a way to add call to action in your plugin. Would I choose alternate action? If yes where do I add the pdf file?

Site should have a call to action that will require visitor to provide their name, email and phone and upon “submit” automatically return a .pdf. Thank you Andrea
