I've been using W3 Total Cache plugin for well over a few years and not only do I like it, but it's saved our bacon on a few sites that have been flooded with traffic due to mainstream press coverage.
W3 Total Cache makes it drop-dead simple to integrate with MaxCDN or several other CDN choices. First time I did it, I couldn't believe I was done so quickly; seemed like it should be much more complicated. :)
Whether you're just getting started implementing caching and/or CDN, or you've been at it for a while, W3 Total Cache is completely worth a serious look.
