Booking form without calendat

Right, i am in desperate need of some help here, because i have spent the last week trying to get some kind of booking form on my website. Through trial and error and still have had no luck.

Before someone tells me about a plugin I have already tried it needs to be the following:

No calendar
When a person ticks a specific box, price is added up for payment.

So for example, person registers their details, they go through the list ticking any items they wish, which add up as part of payment.
they then go to “accept terms and pay” which will take them to a payment page.

I don’t particularly want to have any financial outlay to begin with, but don’t mind paying a percentage of any monies spent.

Any help would be greatly appreciated!
