blank page to start with

Hi. I want a blank webpage, and then I put a form on that page. That is all I want. I do not want all the other wordpress related stuff on that webpage. No menus, no dashboard title bar at the top, and nothing at all about blogs on it either, and no links going anywhere else. I know how to make the contact form, so that is no problem, but trying to figure out how to get rid of all the other unnecessary stuff. Thanks for any help
