Best practise for URL's in posts and pages when moving domains

Just wondering how you are dealing with URL’s in pages and posts that link to other pages or posts when moving your website to another domain or from local to a server.
They’re not dynamic and can’t simple be rewritten to the new and correct URL when you’re changing the URL in the database and/or admin page.
It’s easy with templating due to the (‘template_directory’) tag but what about hardcoded links?
I’ve always been editing the links manually but I’m really wondering if there isn’t a smarter and faster way. It’s the same with uploaded images in posts and pages.
What’s your best practise to address this problem?
