Author and where not posting to database

I’m using 0.3.4.1 and I’m not getting the Author nor the Where to save.
I’ve tried looking at the perfect_quotes.php but for my untrained eyes I can’t see what is wrong (I have disabled all other plugins on my website).
And how do I show the published date on a page?
