Attachment problem in a form – Please help

Hello,
I am currently working on a test website which i have to launch lately.
The page where i am having problem is:

I am using the latest version of Custom Contact Forms on this page while i didnt build this form using the front end builder of CCF but I designed it in HTML according to my ease. Everything was working perfect until i was asked to embed a file upload option in the form and i couldnt do it uptil now. Whats the code for advanced html file upload form? How can i embed a file upload option in such a form?
Please help, The current upload option is of another form which i made with CCF and it refreshed the whole page when it uploads a photo and the text which a user writes obviously disappears.
So there are mainly two problems and the one solution to this problem would be if anyone tells me the code for file upload in CCF. Talking about the custom code.
