Anyone Have A header.php With Meta Tags?

I’m trying to get very specific metatags on my blog and not having any luck with plugins. The keywords are there but when I do a meta tag check from various websites the report is my meta tags are empty.

So can anyone past a sample or real header.php file so that I can edit it and add it to my header.php so I can get meta tags working? Please don’t suggest any plugins.
