All in One Seo Pack wierd issue

I am facing a wierd issue with All in One Seo Pack plugin.

I have set the home page title and the titles of the individual posts on each edit posts section.

But wherever I click, whether posts or pages, only the home title is showing up.

All posts have unique titles, but the same home page title shows on all the posts.

Can anyone tell me whats wrong with it. I have also upgraded WordPress as well as this plugin to the latest version.

wp_head also exists in the header.
