    Hi,

    This functionality is not possible with the free plugin.

    As part of a major update to our premium plugin Product Enquiry Pro for WooCommerce, the plugin would be providing features of a form builder.

    This will let you customise your enquiry form and have these additional input fields that you are looking for.

    The update is expected to release start of next month.
