Adding Header

Hello,
I’m stuck in a rut and I can’t get out…my header here http://www.monkeysaurus.net/amakua_css/index.php needs to be along the top and with the sidebar on the right and the content on the left. I was able to achieve this through this tutorial http://www.transycan.net/blogtest/2005/07/05/integrate/3/ however my results were less than spectacular. I couldn’t seem to get this header to play right if I tried creating a template. I even followed the giraffe tutorials as well but no dice. Can someone point me in the right direction?
