Add Source URL Plugin

I once had this plugin that would put boxs under my post writing section where I could add Text then a URL.

It would look like this after I was finished.

Source: Text

Basically to link my source of my information. Just speed the process up of linking to sources without having to type it all in every time.

It would always put the word “Source” before hyperlink.

Anyway I can’t find this plugin anymore anywhere. Does anyone know of such a plugin? I have tried “Source Affix” plugin which is somewhat the same but it added the hyperlink the wrong way.
