I go to "Ad Buttons", enter image, link, text and ad position and click Create Ad Button and I get a message that the button has been created successfully, but I don't see the button listed below and there are no rows in the ad button table in my database.
So it appears that buttons are not getting saved.

I have not heard this problem before. If you go into the adbuttons settings page and change some settings, do they get updated? Also, you should have one adbutton already setup after installing the plugin, can you disable/enable this button?
