A Submission Question

Hello all and Happy Holidays.. I hopefully have a simple question and I hope I got the right forum..I have a site for models and I have them e-mailing me their info and photos. Now here is my question is there a way to install a submission form on my site where they can upload their photos and fill out their info and hit submit then it comes to my e-mail inbox? p.s i tried the plugin ImageUploader but it made my site go haywire so I had to un-install it.. even if I could just find a way for them to upload their images that would be great as well..any advice?
